Applicants must be enrolled in or planning to enroll in a degree program in occupational therapy or physical therapy at an accredited institution of higher learning. Selection is based on the overall strength of the application.

Applications are available by request. A letter of application, financial aid form, official transcript and two letters of recommendation are required.
