Applicants must plan to major in a health-related field other than sports medicine, non-clinical psychology or social work at a U.S. college or university. They must be full-time college juniors, college seniors or graduate students, and they must have a GPA of 3.0 or higher.

In addition, mail the supporting materials: printed confirmation page from the online application, personal statement, copy of Student Aid Report (SAR) available at www.fafsa.ed.gov and official transcript.
